Arizona’s Spring Turkey Hunter Questionnaire Has Error

Spring turkey hunters are advised that, due to a vendor error, the spring turkey questionnaire that was recently mailed was inadvertently printed on a spring javelina form.
The vendor has corrected the error and will be re-mailing the questionnaires by Thursday of this week. Hunters may wait for the corrected form to arrive in the mail or can go online now (http://www.azgfd.gov/h_f/HunterQuestionnaire.shtml and select the Spring Turkey link) and complete the questionnaire with the username, hunt number and permit number that was printed on the card they just received.
